If you are a member of a Medicaid plan from WellSense, AmeriHealth Caritas or NH Healthy Families, you may be eligible for extra rewards when you take measures to improve your and your family's health. Earn money-back rewards and free healthcare and safety items when you participate in wellness initiatives like attending annual appointments and getting preventative health screenings.     • $25 for a yearly well visit (Ages 3 and over)

    • $25 for cervical cancer screening

    • $10 for flu vaccine

    • $50 for lead screening

    • $25 for completing annual HRA Screening

  • Prenatal and Postpartum Rewards:

    • $10 per prenatal visit (Up to 10 visits)

    • $40 reward and a box of diapers when new moms go for their postpartum visit

    • Sunny Start program (includes free breast pump and other benefits)

    • Infant/toddler safety car seats: If you’re having a baby you can get the seat up to 45 days before your due date

    • Booster seats for kids who are members of our health plan

  • Free Items Annually:

    • New eyeglasses 

    • Replacement batteries for hearing aids

    • One dental kit per individual ages 4-65

    • Bicycle helmets for children

  • Cell Phone Program: 

    • In addition to the federal SafeLink cell phone program, we offer qualifying members in need with access to our own Well Sense cell phone program

    • Up to $200 back for a qualified gym membership

    • 50% back on a fitness tracker (up to $100)

      when you enroll into our plan

    • $10 for completing annual HRA Screening

  • Prenatal and Postpartum Rewards:

    • $10 for completing postpartum assessment with NHHF

    • $100 for Notification of Pregnancy Form (First Trimester)

    • $50 for Notification of Pregnancy Form (Second Trimester)

  • Specialized Care Rewards:

    • Diabetes Care Rewards:

      • $30 for HbA1c test 

      • $30 for Retinopathy screening
