Filling Out New Patient Forms
After you have scheduled an appointment with Amoskeag Health, we will communicate some important information with you through the email address or cell phone that you provided. One of the first things you will need to do is fill out new patient forms. Below, we have some steps and screenshots to help you out.
1) Once your New Patient appointment has been scheduled, you will receive a “Save the Date” email or text message with the date of your appointment, check-in time, and location.
2) Next, you will receive a second email or text message with a green “Fill Forms” button for you to complete new patient forms and medical history prior to your appointment. The process will take about 20 minutes to complete. If you do not complete the forms within 3 hours, you will be required to start again.
3) If you speak a language other than English, click “select the language” in the top left and a box will pop up with all available languages. Most of the instructions on screen will change to your selected language, but some forms will still be in English. Click the green “Start” button to begin filling out the forms.
