Inside the Mental Health Center of Greater Manchester Open Monday 8AM - 4PM, Wednesday 1PM - 4PM, and Friday 1PM - 4PM ProHealth is a partnership between Amoskeag Health and the Mental Health Center of Greater Manchester , working together to support the whole person. ProHealth brings primary care and mental health services together in one place. This makes it easier for youth and adults, ages 12 and up, to get the care they need. Services focus on physical health, mental health, and overall well-being. Our team can provide regular check-ups, screenings, and treatment for physical health needs. We also work closely with your mental health providers to make sure your care is well-coordinated and complete. Learn about the Childhood Response Team (ACERT) at Amoskeag Health. Our team provides specialized support and intervention for children and families facing behavioral and emotional challenges, promoting healthier outcomes and stronger families. ACERT ADVERSE CHILDHOOD EXPERIENCES RESPONSE TEAM What is ACERT? The Adverse Childhood Experiences Response Team (ACERT) is a multi-disciplinary, collaborative approach to addressing the negative effects of childhood trauma. The ACERT model acts as a referral mechanism that connects families and their children to local trauma-informed mental health services and social supports. In Manchester, the ACERT core partnership includes the Police Department, YWCA-NH, and Amoskeag Health who work collaboratively to provide assistance to families, and their children, who have had recent police involvement. This referral program is voluntary and offers connections to a variety of therapies, youth support groups, domestic violence services, athletic enrichment programs, home visiting, and many other child/family-based resources. The ACERT model has been touted as a best-practice secondary health prevention program by former California Surgeon General Dr. Nadine Burke-Harris and has been replicated in multiple other communities across New Hampshire. PRIMARY HEALTH Pediatric care focuses on children newborn to 20 years old and includes their wellness care, immunizations, and development of physical and emotional growth. To support your child, we also can connect parents and caregivers with case management and mental health services if needed. Pediatric Care We focus on treating the whole person and focus on the unique health situations patients between 12 and 20 years old often face. We can screen for depression and anxiety; provide counseling and treatment for substance use; offer information and guidance about appropriate diet and exercise; ensure access to behavioral health therapy; and ensure that all necessary immunizations are up to date. We also offer a full range of education on sexual health including contraception, pregnancy testing, and testing/treatment for sexually transmitted infections (STIs). Adolescent Care Amoskeag Health offers comprehensive, team-based integrated healthcare for adults from all backgrounds and experiences. We address both physical and mental health together! We are considered a Patient Centered Medical Home, which is a model for achieving primary care excellence by providing care in partnership with patients and their families to ensure decisions respect patients’ wants, needs, and preferences. Explore Amoskeag Health’s nutrition and diabetes education services. Our team provides personalized guidance on managing diabetes through diet and lifestyle changes, helping you achieve better health and wellness. NUTRITION & DIABETES EDUCATION Amoskeag Health provides individualized Nutrition consultations offered by a Licensed Dietitian/ Nutritionist (LDN) including nutritional support and guidance for diabetes management, metabolic syndrome, weight loss, and prenatal nutrition (nutrition during pregnancy). We also support prenatal and post-partum lactation support offered by a Certified Lactation Counselor (CLC). Each area of care helps patients identify their unique needs and then create an individualized wellness approach using both diet and lifestyle modifications to support positive nutrition experiences and healthy outcomes. Nutrition We also offer prediabetes and diabetes education and management with a Certified Diabetes Care and Education Specialist and a Nutritionist. These healthcare professionals can explain the diagnosis, blood glucose monitoring and targets, causes of high and low blood glucose levels, meal planning, exercise and weight management, medication options, and how to reduce the risk of complications. LEADERSHIP Gavin Muir, MD Chief Medical Officer/Chief Quality Officer "I joined MCHC, now Amoskeag Health, as a staff physician in 1998. It was and is the only job I’ve had after my Family Medicine residency. I came to New Hampshire from Colorado for the mountains, for the opportunity to provide prenatal care and deliver babies as part of my practice and to work somewhere that would help take care of my student loan debt. After my loans were paid off in 2003, I stayed because I really valued making such a difference in so many people’s lives and because I thoroughly enjoyed working collaboratively with the rest of the health center’s staff in an effort to assist our patients to live their best lives." Gavin is a Family Physician who joined Amoskeag Health in 1998. He is from suburban Philadelphia but has lived in New Hampshire for three decades. He got his undergraduate degree from Princeton University in Ecology & Evolutionary Biology in 1991 and his medical degree from Temple University in 1995 and graduated from his residency program at Southern Colorado Family Medicine in Pueblo, Colorado in 1998. He was a staff physician for his first three years and then started taking on a leadership role at Amoskeag Health. He served as its Medical Director for a decade and then stepped down for two years from that role to focus on the work he was doing with the Elliot Hospital Medical Staff. He served as the Chair of the Department of Medicine at Elliot Hospital for two years and then spent a decade leading Elliot’s Multispecialty Review Committee. He became the Chief Medical Officer of Amoskeag Health in 2013 and has continued to serve in that role. He was a Union Leader Forty Under Forty winner in 2007 and in 2013 he was named the Family Physician of the Year by the NH Academy of Family Physicians. In 2019 he received the Stephen A. Tzinabos award for his leadership and dedication to the Elliot Hospital’s Medical Staff. Last year he stopped doing inpatient medicine after delivering a little over 1,000 babies over 27 years at Elliot Hospital. He continues to see patients in the clinic. Back to Leadership

At Amoskeag Health, we understand that being pregnant can impact your mental health just as much as your physical health. Prenatal Care is a group effort that involves initial screenings by a team that includes mental health professionals, nutritionists, and prenatal nurses. Amoskeag Health partners with Dartmouth Health to provide comprehensive prenatal care. Our skilled providers work together to guide and support you throughout your pregnancy, including your care leading up to and during delivery at Elliot Hospital. Pregnancy & Delivery Information Prenatal Care Amoskeag Health offers Gynecologic Care for healthy female reproductive systems including pap smears, breast exams, STD & STI testing/treatment, and pregnancy testing. We can also provide colposcopy (cervical examination) and LEEP (Loop Electrosurgical Excision Procedures) to identify, test, and treat abnormal cell growth in the cervix. Sexual & Reproductive Health Information Gynecology Services Amoskeag Health has a full-time Community Health Worker (CHW) who works with patients that are due to receive preventive cervical and breast cancer screenings. The CHW assists patients who are unsure about being screened or patients who are experiencing barriers to care such as difficulties with transportation, interpretation, or costs of services. Amoskeag Health is also a Breast and Cervical Cancer Program (BCCP) screening site for the State of NH providing free breast and cervical cancer screening to eligible patients. We have a nurse who assists patients that may be eligible to receive free breast and cervical cancer screenings if they are uninsured. An Amoskeag Health Nurse provides education, support, and assistance to navigate through the program’s options. The cancer screening program is supported by funding from the State of NH’s Department of Health and Human Services BCCP Program and Dartmouth Hitchcock’s NH Colorectal Cancer Screening Program. They can give you a list. Please note: CVS does not accept Wellsense Insurance. Walgreens does not take New Hampshire Healthy Families Insurance, except at the Valley Street Walgreens in Manchester. How to update your pharmacy: Tell the front desk or your provider which pharmacy you want to use. If you need a refill before your next visit, call us and let us know which pharmacy you’d like us to use. Please note that it can take five to seven days for the transfer to happen. You do not need to call your old pharmacy. Your prescriptions will move once we send them to your new pharmacy.
