EARLY CHILDHOOD REGION 4 Supporting Families for Brighter Futures What is Early Childhood Region 4? The purpose of the Regional Early Childhood Structure is to bring together and improve services for kids and families in New Hampshire. It's part of a bigger plan to make early childhood care and education better in the state. The aim is to create a system that prepares children for kindergarten and academic achievement; fosters safe, healthy, and engaged families; and promotes social and emotional health and well-being. The Early Childhood Region 4 is led by the LAUNCH Region 4 Leadership Team who helps support initiatives addressing early childhood in Greater Manchester and Greater Derry. Amoskeag Health and Granite United Way co-lead the LAUNCH Region 4 Leadership Team, with additional participation from several local organizations. Region 4 encompasses Manchester, Derry, and 15 other communities, serving a population of approximately 278,000 people. The LAUNCH Region 4 Leadership Team works closely with both the Manchester LAUNCH and the Greater Derry LAUNCH. These groups consist of advisory committees, collaboratives, and workgroups dedicated to improving early childhood education and support systems. Derry Early Learning Collaborative: Third Tuesday of each month at 10:00 am via Teams. Manchester LAUNCH Family Wellness Collaborative: First Thursday every other month from 9:00-10:30 am in person or via Teams. Community Wide Pyramid Model Leadership team: Third Thursday every other month from 11-12 via Teams. The Pyramid Model is a guide based on evidence-based practices that helps young children learn social and emotional skills. It works with a program's existing lessons instead of being a separate curriculum.     Explore Amoskeag Health’s nutrition and diabetes education services. Our team provides personalized guidance on managing diabetes through diet and lifestyle changes, helping you achieve better health and wellness. NUTRITION & DIABETES EDUCATION Amoskeag Health provides individualized Nutrition consultations offered by a Licensed Dietitian/ Nutritionist (LDN) including nutritional support and guidance for diabetes management, metabolic syndrome, weight loss, and prenatal nutrition (nutrition during pregnancy). We also support prenatal and post-partum lactation support offered by a Certified Lactation Counselor (CLC). Each area of care helps patients identify their unique needs and then create an individualized wellness approach using both diet and lifestyle modifications to support positive nutrition experiences and healthy outcomes. Nutrition We also offer prediabetes and diabetes education and management with a Certified Diabetes Care and Education Specialist and a Nutritionist. These healthcare professionals can explain the diagnosis, blood glucose monitoring and targets, causes of high and low blood glucose levels, meal planning, exercise and weight management, medication options, and how to reduce the risk of complications. This program is offered at no charge to all Amoskeag Health patients with children 0-8 years of age. LAUNCH is a clinic-based program, which means our "LAUNCH Navigators" work directly with parents and caregivers to discover and navigate local resources for childcare assistance, food access, housing assistance, and more. We provide referrals and set up ongoing support to make sure you have everything you need. Our Ready Set LAUNCH program is for newborns and their families who live in Manchester and give birth at Elliot Hospital. Newborns and their families are eligible for this one-time home-visiting service! Learn More Free Developmental Screenings Completing the development screenings during your child's first year of life helps you understand your child's milestones, learn what they are focused on and what skills you will see developing soon. Screening can be completed in 30 minutes. Results over the phone or in person. Activity ideas provided to support your child's development. Manchester LFWC attends community-based events with information on child development, community resources, programs, and information in collaboration with partners The Early Learning Collaborative Early Learning Collaboratives offer a space for childcare centers and family-based center directors to convene. At Amoskeag Health, we understand that being pregnant can impact your mental health just as much as your physical health. Prenatal Care is a group effort that involves initial screenings by a team that includes mental health professionals, nutritionists, and prenatal nurses. Amoskeag Health partners with Dartmouth Health to provide comprehensive prenatal care. Our skilled providers work together to guide and support you throughout your pregnancy, including your care leading up to and during delivery at Elliot Hospital. Pregnancy & Delivery Information Prenatal Care Amoskeag Health offers Gynecologic Care for healthy female reproductive systems including pap smears, breast exams, STD & STI testing/treatment, and pregnancy testing. We can also provide colposcopy (cervical examination) and LEEP (Loop Electrosurgical Excision Procedures) to identify, test, and treat abnormal cell growth in the cervix. Sexual & Reproductive Health Information Gynecology Services Amoskeag Health has a full-time Community Health Worker (CHW) who works with patients that are due to receive preventive cervical and breast cancer screenings. The CHW assists patients who are unsure about being screened or patients who are experiencing barriers to care such as difficulties with transportation, interpretation, or costs of services. Amoskeag Health is also a Breast and Cervical Cancer Program (BCCP) screening site for the State of NH providing free breast and cervical cancer screening to eligible patients. We have a nurse who assists patients that may be eligible to receive free breast and cervical cancer screenings if they are uninsured. An Amoskeag Health Nurse provides education, support, and assistance to navigate through the program’s options. The cancer screening program is supported by funding from the State of NH’s Department of Health and Human Services BCCP Program and Dartmouth Hitchcock’s NH Colorectal Cancer Screening Program.
