Manchester Community Health Center Receives Award for Senior Care
Expanding Care for High-Risk Senior Women

MANCHESTER, NH: Manchester Community Health Center has been awarded a $40,000 grant from the Mary Gale Foundation Fund of the New Hampshire Charitable Foundation to expand senior care services for women 65 and older.

Funding from the Mary Gale Foundation Fund will directly strengthen the Complex Care Management Program currently assisting low-income women 65 and older in the Greater Manchester area. This proactive, evidence-informed, patient-centered program focuses on improving the health outcomes for this high-risk, uninsured or underinsured elder population. MCHC will now be able to expand care management support services to help high-risk elderly patients proactively manage chronic illnesses to maintain a healthy lifestyle. Additionally, MCHC will introduce PRAPARE, the Protocol for Responding to and Assessing Patients’ Assets, Risks, and Experiences. This tool will allow the Complex Care Nurse and Social Service Case Manager to better document and manage elder patient needs and link patients with community services and health education.

This funding allows us to enhance support services for these women who often experience high rates of chronic disease including hypertension, depression, diabetes, and obesity and require additional assistance understanding their complex health needs.”
